If you would like the GP Practice to contact you by this means please sign up by making sure we have your up to date Mobile Phone number. This can be done at reception by filling out an up to date contact form or by filling out a change of address form - Link on Change of Address Information page 4.
If you agree to the GP Practice contacting you via test message to your mobile phone, the GP Practice agrees to adhere to the following:-
- The mobile phone number will only be used by the GP Practice and will not be passed to any other parties.
- No personal details will be included in the message to identify you.
HOUSE OF CARE APPROACH TO ANNUAL REVIEW
The practice are introducing a new model of delivering annual reviews to patients with chronic disease. This model has been introduced across the UK and involves a two appointment process. Measurements will be gathered by our Health Care Assistant and the results will be sent to the patient before the second appointment with the Nurse. We will role this approach out over the next year commencing with our diabetic patients. Please note you will be invited to attend for this new style of annual review by letter as we have done in the past. More information on this approach can be found at:
MEN C VACCINATION FOR UNIVERSITY STUDENTS
The practice have been directed this year to provide a time-limited catch up programme for first time university entrants under the age of 25. This will run until 2017 as from them entrants will have routinely received their adolescent vaccine booster.
Should you wish to have this vaccine please contact the surgery to make an appointment.
August is change over time for our trainees. This year we say goodbye to Dr V.Young. We would like to thank her for her contribution to the practice over the year and wish her every success in the future.
We welcome Dr Z.Chandy who will be with us for a year and we look forward to working with her.
All men and women aged 50–74 are invited for bowel screening. Bowel screening involves taking a simple test at home every two years.
We strongly encourage you to take this simple test. In Scotland the screening programme will prevent at least 150 deaths from bowel cancer each year

We no longer keep a supply of self certificate forms in the surgery. You can download a copy of the form here sc2.pdf or request one from your employer. This form should be used for periods of absence up to 7 days.
If you require a doctors fitnote (formerly sickness certificate) following a period of absence greater than 7 days please contact the surgery.
Please note a change to health board procedures states that where the absence has been initiated by a hospital doctor, either as an inpatient or outpatient, it is the hospitals responsibility to issue the fitnote. Please get in touch with the appropriate department.
PATIENT CONTACT DETAILS
We are looking at ways of improving keeping in touch with our patients. As a result we are collecting patient mobile phone numbers and email addresses. This will allow us to send reminders of letters via these methods in the future.
