Please let us know as soon as possible if your address changes. If you have moved out of the G61 and G62 area you will be asked to register at another practice in your catchment area.

If you change your telephone number please advise a member of the team at the surgery. Failure to do so may result in us being unable to contact you quickly should we receive urgent information to pass on to you, for example about investigations or blood tests.
